Need a SIMPLE Palm app to keep track of Pt's for PCT job...Anyone?

I'm working as a PCT/PCA during the summer and probably a little when school starts. I would love to find a simple Palm based app that I can use to keep track of my patient vitals, I/O, etc.

It would great to have something that I can jot down vitals, I/O, and a few misc. notes (cbg, foley, etc.). Pt. would be identified by room and bed.

Just need something that can serve as the sheet of paper that isn't going to be all mangled by end of shift when I'm trying to enter everything in the electronic patient record (EPIC).
