Which health care members are involve in the decision making when it comes to withholding or withdrawing treatment within the ICU setting?
Who is the Senior member of the team who takes responsibility for the decision with rationale.

It is a team effort where I work (OHIO USA) - family members and physicians and pastoral care usually meet and discuss issues and end of life care when the time seems appropriate. The primary care physician usually writes the orders and they are carried out.. anything else? Do you have Palliative Care..or Hospice?

I believe in NY State, the family, physican, socialwork and nursing sit down at the table with the Ethics Committee and decide on what happens, taking into consideration care plans
, the family's wishes, wishes that the pt make have made known, hospital policy and other contributing factors.

Usually it's the patient's family, the attending doc, consultants and nurses at our facility. It's not usually a problem unless one of the above mentioned folks has a problem with it. Then it goes to Ethics. It doesn't routinely go to ethics.
