Tis true, soon you won't need a work sheet the way you do now.

My husband help me with one using Excel. He's the computer geek, so it really turned out nice. Now my unit uses a MAR worksheet that is distributed at the beginning of each shift. So, since we started using the computer for so much of our charting I haven't used my sheet.

Thank you for these report sheets. I'm a new graduate and new nurse, I'm having a difficult time trying to organize myself during report and this should help a great deal. I'm going to try them tomorrow when i go in.

I've got one that I've been working on too, but I'm not sure how well it will work for ICU. I'm currently a 2nd year RN so I'm floating all over, and it seems to work well for that. I'll need to make more room for multiple vitals I'm thinking once I hit ICU in a couple years (sooner the better).

Honestly, I just go by memory. I work with people who lose their minds if they misplace their "memory sheets". I orient a lot of new nurses. Some make up sheets based on body systems and some just jot down important notes. Meds are on the MAR, vent settings are something I remember, vitals, reding ect. If you are doing in depth assessments and making a mental not of what is going on it is usually to remember ..........my drips are set on x, my vent is set on y, my patients vitals and rhythm are x, and so on. We use shift kardex sheets for dates of procedures and to update drips during report sometimes. We also do 12 chart checks as well.
