I'm his brother but thanks for the kudos anyway. I decided to go with a watch seeing as he is horrible with keeping time.

I would go with a wristwatch rather than a lapel watch. The only people I've seen wearing them are female nurses...

A gift card and a really personal letter expressing your pride and love for your brother may be a better idea since distance is a factor. A phone card to call you too. You are very nice to want to do something special for him.

A little late chiming in, but a watch with a second hand is nice when you're counting respirations or taking pulses.

Pens are a popular graduation gift, and nurses do need pens--but a box of cheap pens might be better than one really nice one. They get lost, borrowed, and contaminated.

A good bottle of whisky can be nice for after work...
