    I seen on a thread that you use to be a former surveyor and had a question. We had a pt that was found holding on to the side rail on the floor, had no injuries, assessed by 2 nurses. Pt was too large to get up so we called help. About 5 minutes later pt stops breathing and dies. He is was already very sick. The doctor was not surprised and had seen him that same morning. State was already in the building so they are now investigating. How does this process work. Thanks
