    Same thing with home care. Patients don't get the number of visits they were told they'd get that week. I'm the one who gets blamed even though I don't make the visit schedule but the office makes no effort to get in contact to explain missed visits....

    P.S. If it's been 30 days since the actual surgery (i.e. if the patient went to rehab for long period of time after hospital) then we're told to put "New Epith"...I guess a 30 day old incision (WNL) is then considered healed.

    Our office requests the same documentation for knees/hips/surgical incisions ... EVERYTHING at a SOC is considered non-healing. Why are you uncomfortable with this? It's just baseline documentation. We're given the same rationale...need to see som...

    Our charting (all computerized) is to be locked down and transmitted by midnight after the visit was performed. Granted there are times where we slack and do it the next day but it is the expectation that it's done by midnight.

    For the short period of time we used them (they showed no significant improvement in our CLAB rates) we only put them on the PICC/Subclavian/Port caps, I never saw them used on the actual IV tubing but that was just what I saw.
