I have been an MDS nurse for 3 years. In the last 1 1/2 year or so, I have only worked the floor about 4 or 5 times, and that was when I was on call. We take call for a whole week at a time, every 5 weeks. I find that so many of the nurses have ...
  2. MDS Questions???????????

Hi Mermaid, since I know you & I work for the same company, I will tell you what I do. In datawarehouse on Knect, I print the 100 day scheduler for Medicare residents. It gives you the range for all the assessments. I pick all the dates for the ...

  4. Care plans...what is and isn't necessary

I agree with the previous posts. I make sure all my Raps are covered, and even then I combine problems as much as I can. I also care plan anticoagulants, dig, etc. OMG, the care plans would be 500 pages long if you careplanned every med & dx!

  6. throw in the towel with MDS

I have been a MDS coordinator for 3 years. It took me a good 3 months to really know what I was doing, and another 3 to get comfortable. It is too bad that the other MDS coordinator is not willing to help you. This job is not for everyone! Tomorro...
  7. Creams and ointment...foley care..

We wrote a policy a couple of years ago that states cnts may apply some topicals to unbroken skin. So if there is an open area involved, the TX nurse or nurse assigned must do the applying. Also, we don't get orders for barrier ointments, it is on t...
  8. Difficult Post/CVA Patient

Has she been assessed for pain? I often find that to be the cause of yelling, poor po intake, refusing therapy, etc. ~Robin
  9. Admission Nurse

I do the admission process from start to finish. I do the pre-admission screening, list diagnosis for Health Information to code. If they are not in for some reason, I have to find the ICD-9 codes. I do a 6 page nursing assessment, plus the suppleme...
  10. Pre-pouring meds

OMG this PHTLS person is obnoxious! Get over yourself! I have worked in LTC & acute hospitals, and have never pre-poured meds! When I worked the floor, I developed a routine. In LTC, you have the luxury of knowing how your residents take their me...
  11. Anyone else here do pre-admission screenings and marketing?

My facility has a full-time admission director. When she receives a referral, the business office manager screens insurance, & I reveiw the clinical info. If I question if we can take a patient clinically, I have the DNS review it. Amen to the pe...
  12. Facility Soap Opera

That girl needs to seek employment elsewhere! She is putting the rest of the staff, residents, and visitors at risk. I would be afraid to work my shift!
  13. MDS staffing

OMG I am glad that I don't work for your DON. The DON I work for is always asking everyone what she can do to help! I wonder if your administrator & DON have ever heard of a DAVE audit! If you don't have the time to make sure the MDS is filled ...
  14. We go over what needs to be charted at our weekly Medicare meeting for the SNF residents, and the skilled charge nurses come & take notes. But it is still an ongoing problem. CapeCod is absolutely right in that you need to chart according to what...
  15. MDS staffing

I work for Kindred Healthcare. Yes, the test was intense. We went for 2 days. The first day we covered the RAI manual page by page! The second day was the test. I am glad they did it, to make sure everyone is doing the mds correctly. I was named MDS...
