Work Sheets and Getting Report - page 2

Hi, I am a new grad and a new nurse I need help finding a worksheet that will help me quickly and accurately take report. I have tried making my own but have been unsuccessful. Can anybody help?... Read More

    I sent a worksheet that I made up for us to use as a report sheet and pass it to next shift
    you can do the same for your worksheet but delete the middle out of it and put several on a page for a daily worksheet if you want.

    I am a nursing student and I am having trouble finding a way to organize all the information during report so i don't miss anything. I have been trying out a variety of different worksheets and i was wondering if some people could send me their 'brains' so i can see how other people do and combine it into something i could use

    Hello...
    I am a Second year Nursing student... in my last semester, soon to be a senior! Hooray!!!

    However, I do not yet have a brain... hehehe " if i only had a brain"

    I am having alot of trouble keeping organized...
    Does anyone have anything they can send me such as a report form or any kind of lists ?

    If you have computer charting or computer generated kardexes you can run a copy of your pts kardexes,staple them together and use a highlighter for important info.such as allergies,IV fluids,labs due,etc..Take report in 1 color ink and write your shift assessment,vs's, etc in another color.You have everything you need right there with you at all times.(These also have all the meds listed.)Add new orders to your kardex copy as you go and its a breeze to give report from.It's more difficult if you're in charge though because its just too much paper to carry with you.Hope you find this helpful.

    I too am looking for a worksheet as well. I have a couple that i've been using, however I dont seem to have enough space by the end of my shift.

    I am a 4ht yr student in my consolidation on an Leukemia/Lymphoma unit. The unit can be very hectic once the new, and continuously changing, dr's orders come through, with multiple antibiotics, boluses and blood product transfusions.

    so i would really appreciate it if someone out there ANYONE who has a form thats workable!

    I made one for when i charge (which is all the time) and its mostly for meds and stuff that needs to be collected and what i am responsible for

    I work on med surg and I find it very difficult to fina a worksheet that is good too. Although I think the best way to organize your sheet is by dx sx diet activity IVF HX ALLERGIES then have a space for comments...

    on my last placement they used preprinted forms
    name age reason for admission pmh, allergies dnr status, social, diet mobility continence, and comments live iv ab, iv fluids etc.
    i would add jobs etc on the back using tickboxes
