JCAHO Alert: Tubing misconnections—a persistent and potentially deadly occurrence

My God I hope that none of these nurse are my friends. I would be so angry with them. We as vent subacute nurse use alot of tubes and I don't know how they could get them all mixed up.If you don't know what you are doing ASK!

Wow. I had never stopped to think about it. We give our Histussin in syringes and usually my people either like to squirt it into their mouths themselves or have me do it if they are elderly and feeble. However, I think from now on I am going to squirt it into a cup. Thanks for the tip.

OMG, I can't believe that, this is good information, and will sure to label lines more carefully. As someone posted earlier, MOM is white, and why wouldn't you stop to think before giving that, but Propofol is an IV med that is also white. However, if you have both MOM and propofol in syringes, for some reason. CLEARLY label those meds. Some of those things are scary, and I hope nothing like that happens to me. I am not perfect by any stretch of the imagination, but I always follow tubing that I'm changing or connecting into, from point of orgin back (or vise versa), to BE SURE, that I am connecting or changing the right line.
