  3. Therapy Start Date: "This is the date the initial therapy evaluation is conducted regardless if treatment was rendered or not or the date of resumption (O0450) on the resident's EOT OMRA, in cases where the resident discontinued and then resumed therapy." (RAI Manual, page O-16)

If the first evaluation/treatment did not occur until 6/11 then that is the start date.
  4. Unless Im reading the manual wrong, I believe it reads "the therapist time spent on documentation or on initial evaluation are not included."
For the second part of your question, I check off "none of the above" only if the resident has no dentures and no teeth.
  5. "the therapist's time spent on documentation or on initial evaluation are not included" relates to counting the minutes (pg O-17), not to date determination.

Additionally, I would like to point out that there must be MD orders (proper documentation) for a Start of Therapy Date to be entered, even if there are no minutes counted (evaluation only).
