    How are you guys filling it out?? Does your computers fill out correctly?

    We did ours-- and it is NOT correct at all.. ;(

    So we have to hand update it...

    Usually if all MDS are current and transmitted, this form is correct. Just if a sig change has just occurred or someone passed, it can be wrong. always make sure things are correct before handing this over to inspectors.
