Good nursing assessment tools for new admissions


Anyone have any good admission assessments out there that help you capture better rug scores?? I think our standard Briggs forms just dont cut it well half the time cant even get the nurses to get a weight very fustrating and seems like I spend half my time getting the information myself even the weight most of the time. Hugs to all you hard working MDS People!!
