Discontinue Medications Order?


hello, silly question if MD order discontinue bactrim medication do you count that as an Physician orders in section P8? or not since I already count the order when bactrim was initially order, would it be double if i count the dc order also?

The Intent is - "To record the number of days during the last 14-day period (or since admission, if less than 14 days ago) in which a physician has changed the resident’s orders." If it occured in two separate days, then count as 2.

If it was written on 2 separate days it is counted as 2 separate orders.
