Confused about Section M


I'm having a hard time understanding "scabs". Are they to be coded as a stage 2 pressure ulcer if they occur over a pressure area? How do you determine if they're caused by pressure or not?
Also, what about a "paper tape burn", would this be coded as an "open lesion"?

Any help would be greatly appreciated.

A scab is coded a St II, per RAI p3-159, that is, if the skin ulcer is due to a circulatory problem or pressure. Otherwise it will not be captured in M1 but in M4.

If the cause cannot be determined, it would essential the clinical staff unanimously agree on how to label the skin problem and code the MDS as they see fit. What matters more is your team's plan of care.

A tape burn is usually a reaction to an irritant, thus, a rash. Albeit, a tape that is tightly applied for a long duration may lead to a pressure sore.
