    Do you invite all of your residents to careplan meeting? We do not invite those that are severely cognitively impaired. We do document why they were not invited. We were cited on our last survey because not all residents were invited.

  3. If the resident is incompetent, the regs §483.20(k) require you to involve the resident's family, a surrogate, or a representative in the development of the care plan.

    This does not necessarily have to be a formal face-to-face meeting w/ the team, but a note to have notified/invited them to one, is beneficial. Documented ongoing communication (by telephone, email, direct interaction during visits) w/ the resident's designee regarding the care provided and soliciting their input is evidence of their involvement.
