Orientation Checklists for Nurses/CNA's

Hello all,

I am a relatively new DON in the skilled rehab unit of a LTC facility. I am in need of orientation checklists for CNA's and LVN's/RN's. Not wanting to reinvent the wheel, so I am curiousl if anyone who already has forms like these is willing to share.
