    When everyone is completing their CMS 672 - Census and Condition Report, for F143 Advanced Directives, is anyone counting the POLST as one of the items? I know it states a living will or POA but, to me the POLST would fall under this category.

    We were supposed to start using the MOLST, but the forms aren't available, so we're not counting it on the 672.
