WIA application for training funds

I need an LPN to answer these questions please. Its part of my application to do research by interviewing 2 LPN's and 3 companies that hire them. Please provide the following information:

Your employer name, your job title, how long you have worked in this field, how long have you worked for this employer, your salary range (i.e., $20,000-$30,000), what is the best and worst thing about your job?
