Medicare and HMO charting


I just started a position at a ltc/rehab facility and I need to know how to chart for medicare and hmo clients.
Is it that we focus our charting about the diagnosis condition for medicare and about therapies for hmo? This is just a guess from what I heard in orientation, but I am just not sure. I will also look on the web but it would be helpful to me if anyone can help me here. If you could give an example. Thanks!!
