Care plan question

i am a student and working on my care plan and i had found this one in a pts chart. it was written by one of the nurses and i don't think i can use it because i can't find it on the nanda list

here it is and if you can help me reword it so it is correct, that would be great because i am having a tough time

assistance needed with adls r/t impaired memory and cognitive skills, impaired balance and weakness, unsteady, abnormality of gait.

if anyone can help me fix this so i can do it correctly, i will be grateful.

From what I can see, there is: Self-Care Deficit, Bathing/Hygiene r/t impaired memory/cognition as evidenced by (insert evidence). Self-Care Deficit, Dressing/Grooming r/t impaired memory/cognition as evidenced by (insert). I didn't see a NANDA approved diagnosis that covers all the above, but that doesn't mean there isn't one. Word of caution: Memory, Impaired is a nursing diagnosis itself, and some (my) instructors didn't like for us to have a nursing diagnosis in a nursing diagnosis. The other one I see is: Falls, Risk For, r/t impaired balance as evidenced by unsteady gait. ALWAYS run them by your instructors first, as they know what they're looking for. I took these from my book from school. I can't take credit for just knowing these. I always struggled with them, too. Anyway, I hope it helps.

Oh! Ewww. I hope they work for you. I did take them right out of a NANDA book, so at least there's that much going for you!

Any suggestions for Spiritual Assessment and Nursing Diagnosis for "Agnostic" patient? I've searched on-line w/o luck.
