Home Health RN Team Leader
Hospice by the Bay's newly aquired Home Health Services company, InCare, is a full service Home Care Agency. We provide a wide range of specialized services tailored to meet the needs of every individual on a personal provider to patient basis. InCare Home Health Services improves the patient's quality of life by enabling individuals to stay in the comfort and security of their own homes during times of illness, disability or recuperation. We reinforce and supplement informal care by educating the patient's family members and friends about the care giving process.

We are seeking an experienced Registered Nurse to serve as a Team Leader in our San Mateo location.  This Home Health RN will be responsible for the imple­mentation, delivery, coordination and supervision of services for a team, in compliance with organization policies and procedures and applicable laws and regulations.

Essential Duties & Responsibilities includethe following but other duties may be assigned:
  • Supervise Field Registered Nurses and their required clinical activities
  • Oversee the implementation of a physician’s Plan of Care, coordinating activities with resources
  • Manage, align and select the skilled medical professionals and services provided by evaluating the need of the client, feedback from the family and interaction with others working on the case
  • Oversee client records and file maintenance
  • Provide staff development opportunities/in-services within their specialty and in general on a regular, ongoing basis maintaining documentation of said requirements/classes. Stay in-tuned on best practices and educate staff on their techniques
  • Stay informed on current caseloads providing an effective balance between services offered, services required and staffing resources 

The requirements for this position include:
  • 2+ years of nursing experience in a home health setting
  • 2+ years of supervisory experience in a home health setting
  • Current license as a registered professional nurse in the State of California
  • Experience with and high level of comfort using an Electronic Medical Record system
  • Strong working knowledge of regulations associated with CHAP
