Description:
Works collaboratively with physicians, staff and other health care professionals to provide care coordination for identified patient populations with multiple chronic or high-risk conditions. Incorporates evidence based best practice guidelines to ensure high quality standards are maintained across the continuum. Engages and empowers patients to strive for optimal levels of health and wellness by active participation in their disease management. Responsible for coordinating self-management support and maintenance of disease database for the specified patient population. Communicates and coordinates with the healthcare team for benchmarking, tracking and measuring optimal patient outcomes and report findings.

Qualifications:

Required: Graduate of an accredited School of Nursing

Required: Must be currently licensed
