Founded in 1975, Hospice by the Bay was California’s first hospice and was the second in the nation.  The nonprofit currently serves children of all ages, adult patients and their families in the counties of Marin, San Francisco, San Mateo and Sonoma, and the cities of Napa, American Canyon and Vallejo.  A pioneer in the field for more than 40 years, HBTB affiliated with UCSF in 2015.

Hospice by the Bay is seeking an experienced Registered Nurse to serve as a Marin County Team Leader.  This role is responsible for the implementation, delivery, coordination and supervision of transdisciplinary team services for assigned team, in compliance with organization policies and procedures and applicable laws and regulations. All clinical supervision is provided by a supervisor or specialist of that same discipline. The ideal candidate will have strong assessment and leadership skills.
Essential Duties & Responsibilities Include:
  • Supervise members of the transdisciplinary team, ensuring that all disciplines are provided appropriate clinical supervision by a supervisor of their discipline.
  • Supervise the planning, coordination, and delivery of the transdisciplinary team interventions.
  • Assign patient/family to appropriate staff member(s).
  • Participate and provide leadership to the team in regular transdisciplinary team meetings; the development, review and revision of the patient/family plan of care; facilitation of problem solving sessions; and delivery of team support.
The requirements for this position include:
  • 2+ years of nursing experience in a hospice setting
  • 2+ years of supervisory experience
  • Current license as a registered professional nurse in the State of California
  • Experience with and high level of comfort using an Electronic Medical Record system
  • Strong working knowledge of regulations associated with CHAP
