Founded in 1975, Hospice by the Bay was California’s first hospice and was the second in the nation.  The nonprofit currently serves children of all ages, adult patients and their families in the counties of Marin, San Francisco, San Mateo and Sonoma, and the cities of Napa, American Canyon and Vallejo.  A pioneer in the field for more than 40 years, HBTB affiliated with UCSF in 2015.

We are seeking a Nurse Practitioner to perform face-to-face encounter with hospice patients to determine continued eligibility of the individual for hospice care consistent with Hospice by the Bay policies and procedures and in compliance with organization policies and procedures and applicable laws and regulations.

 

Essential Duties & Responsibilities Include:

No more than 15 calendar days prior to the 180-day recertification and each recertification period thereafter conduct face-to-face visit with hospice patients to gather clinical findings to determine continued eligibility for hospice care.

Document face-to-face encounter with the patient, including the date of that visit.

Provide the clinical findings from that encounter to the physician who is considering recertifying the patient.

Demonstrate awareness of and sensitivity to hospice programs and services.

Maintain confidentiality according to agency policies and procedures. 

Skills & Qualifications:

Strong working knowledge of nursing practices, preferably in a hospice care setting.

Ability to read and interpret documents such as policies and procedures and clinical documentation.

Ability to write clear and comprehensive clinical documentation and routine correspondence.

Ability to effectively present information in one-on-one and small group situations to clients and other employees of the agency

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.

Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Excellent oral and written communication and interpersonal skills, including the ability to listen effectively, to select and use appropriate communication methods, and to present ideas and information effectively, both orally and in writing.

Demonstrated ability to effectively work with patients/families dealing with life-threatening illness and death including sensitivity to interfaith and multicultural issues.

Ability to view every action and decision in terms of client impact and to understand and anticipate client needs.

Ability to manage multiple tasks and priorities to conclusion.

Education/Experience:

Masters of Science in Nursing required.

Minimum of two years of professional nursing experience with at least one year in an acute care, home health or hospice setting.

Certificates, Licenses, Registrations:

Advanced practice Registered Nurse with active Nurse Practitioner license.

HPNA and/or PHN certification preferred.
