Job Description:
Maintains an environment of safety for patients, self and others. Competently assesses and documents the patient's biophysical, psychosocial, cultural, spiritual, safety, educational and age specific needs using the nursing process. Implements the nursing plan of care on assigned patients, and evaluates patient's status, nursing instructions, appropriateness of the nursing plan of care, and outcomes of the care provided. Designs and implements an individualized comprehensive plan of care in collaboration with other team members that is consistent with the medical regimen, nursing standards of practice and regulatory guidelines. Updates the nursing care plan, as the patients needs change, when the patient achieves goals, and documents nursing instructions given. Develops and revises the Home Health Aide Plan of Care as appropriate. Initiates a patient/family/caregiver education plan based on the individualized needs of the patient/family and discharge plans; as prescribed by the physician and/or home health agency policies, procedures and care guides. Maintains confidentiality of information; notifies patients and Team Leader of schedule changes; completes and submits paperwork in accordance with agency policies and procedures. Communicates effectively with physicians, patients, families, caregivers, staff members and community resources. Participates in chart reviews to identify and provide intervention for potential risks, performance improvement opportunities and ensure quality patient services and outcomes. Participates in performance improvement activities and other committee meetings as assigned. Assumes primary responsibility for the overall coordination of patient's home care needs. Documents patient's status maintains confidentiality of client information per agency policies and procedures. Completes and submits documentation in accordance with agency policies and procedures. Develops an individualized plan of care utilizing the nursing and medical diagnoses. Collaborates with other health care team members in formulating plan. Provides in-home psychological education and supportive therapy. Evaluates patient's status, nursing instructions, appropriateness of the nursing plan of care, and outcomes of the care provided. Participates in RHS performance improvement activities, psychiatric clinical supervisory meetings, and other committee meetings.

Job Qualifications:
Experience Minimum of 1-year Medical-Surgical experience and home health experience preferred. Additional experience is required with most specialty areas such as two years of mental health experience.

Education Bachelor's degree (BSN) from an accredited School of Nursing preferred.

Licensure Must have NC RN Licensure, valid NC Driver's License, and BCLS
