Visa Screen Certificate / CGFNS

hello there! im new to this site.
i would like to know how I can renew my visa screen. I passed CGFNS last 2004 and combined it with my IELTS last 2005,thus gained my visa screen. but as we all know,there is still this retrogression.My visa screen will expired next year. please help me...I dont know what to do to renew it.

hi guys, recently i have passed the NCLEX-RN Exams for Vermont last July 27, known the results 48 hours later via the Internet and i am also finished taking the IELTS Exam. Eventhough, i have passed the NCLEX, i still don't have a registration number and a license from the state of Vermont. Here are my questions:

a. on the question statement "did passing of this exam (NCLEX-RN) lead to a license being issued in the same State/Province and Country?" should i answer "No" on that question?...because eventhough i passed my NCLEX Exams for Vermont...i cannot still process my license because my confirmation letter that i have passed the exams hasn't arrived yet.


b "If licensed in the United States, Canada or Australia, please list the state or province in which you were licensed

_______________________________ Registration Number_______________________"

Should i leave this statement blank?...because i still dont have a registration number and license from the VErmont BON


I'm very confused on this statemets and these are the only questions that hindering me from passing my VisaScreen Application...I just want to beat the November 15 deadline on my IELTS exams taken 2 years ago...

You should wait for your license number from vermont. According to the CGFNS all the informations should be provided, if not, they will return the application form thus prolonging your application. Try to visit vermont website and probably your registration or license is already posted there or try to call them to speed up the process.

guys, I'm confused about this statements in the Application Form

A. About the Authorization to Release Information...I don't want to disclose any information about my files sent to CGFNS/ICHP...should i leave this behind or should i sent this with the rest of my application forms but leaving it completely blank?...

B.About the Credit Card Payment Form...do CGFNS allowed a third party to pay for my fees?... (I want it to be charged to the credit card of a friend of mine since i don't have one.) and if this allowed should it be sent with the rest of my forms?...because it is printed in this page "detach this form if payment only if payment is being made by a third party"

C.on the request for Academic Records...

between (dates of attendance) ___________and____________

-Should i fill out the complete date or only the month and the year that i am enrolled in college?...

-Aside from that the complete name of the state (Negros Occidental) which i reside in....cannot be completely jotted down on the space being provided for the NAME OF STATE/PROVINCE...what should i do on this part?....

D.About the photographs...the total photos to be sent in passport size..is

1 Photo being pasted in the Photo Identification Form with a complete signature of my name in it's front and 2 Passport Photos being signed at the front and the back of the pic with my signature only (not my full name)...am i right?

Hi. Do not include the authorization to release information form, that is usually for a third party such as an agency that assists you. You can use the credit card of your friend just fill up the form properly(Payment with credit card form). You can write the province in two lines just to fit, as long as it is readable. Lastly, you will include only one photo, passport size with your signature using your full name. The signature should match with the other forms that you will submit to them.

You mean to say,that my signature using my full name is the one that I will use to fill in every signature entry in all of the application forms?....


Also I'm wondering if i made the correct entries in this questions:

A. In which other country or countries do you currently have, or have ever held a nursing license? I jotted down Philippines in the entry form or Should I leave this blank it's because I'm only licensed to practice Nursing in the Philippines only, apart from the U.S.

B.If you passed either SBTPE or NCLEX-RN,Please list the date and location where you passed the exams

Month 07 Day 27 Year: 2009 State/Province _____?____ Country______?_______

In the State Entry...should i put Manila it's because that's where I took the NCLEX Exams or should I put Vermont because that's where I applied for my exams?....

I'm very sorry if I am a bit of a nuisance here asking these type of questions it's because that I just simply wanted to finish my Visa Screen Application with the less time and effort so that I could return to my job as a nurse in our hospital...My Supervisor will not allow me to return to my workplace if I cannot finish this forms...She wanted me to focus solely on my work as a Staff Nurse not on these applications when I return to my job...

Yes you are correct. You have to use your full name signature in all the forms for your application in CGFNS, that means you should also include your middle name.

A. You have to declare that you are also a licensee in the USA. So, you should write Phil and USA
B. Write "Vermont" because that is where you had your license and "USA" for the country.

I don't see any problem working while applying for a Visascreen. You just need to fill it up with what they require. Anyway, if you need any help, don't be hesitant to ask again. I'll be glad to answer your queries.

By the way, my application has been processed already and the only thing that the CG is waiting for is my verification of license here in Phil. I have only provided a photocopy of what they require, I did not notarized it. So far, no feedback from them that my documents are not accepted. But they have done their initial evaluation already, so probably its acceptable.
I am also a licensee from Vermont.
