US RN verification to Queensland

Hello I've been preparing all the documents for the application. I am currently holding California and Texas RN licenses. How do I send license verifications to Queensland council? I think I am supposed to send the verification request forms to the two states, and then the state boards will send my verifications directly to the nursing council in QLD, but where do I find the forms? Thanks in advance!
