It is a matter of practice, practice, practice.
Sounds like maybe you are in the Oklahoma City area, so there should be lots of resources there.
See if your public library has a literacy program--sometimes they have volunteers who come in just to have conversations to help spruce up accents, pronunciation, use of idioms, like that.
Try to speak only English, even if you are at home with your family. You can all speak English together--and correct each other's pronunciation.
It's clear that you are thinking in English and understanding English, even the complicated stuff that will be on nursing exams. Your issue is being understood by people with whom you need to communicate: patients, colleagues, physicians, etc.
