request for validation of registration

Hello, I want to take NCLEX and before that I have to my degree and registration to be evaluated by Credentials Evaluation Services. There is one form for “request for validation of registration for credential evaluation services” this form needs to be filled by registration office that gave me license. I am wondering if I have to send this form to CNO? Does anybody know?

The form needs to go to the nursing board where your Canadian license was issued.
