You need to fill in only the one page for NY, and then the two applications for CGFNS. One for Visa Screen Certificate and one for the Special Credentials Verification for NY. CGFNS will obtain all of your needed documents for you, you only need to sign releases for that. (The NY BON application actually has five parts but you only need the one page if you are a foreign grad.)
