need HELP ON CVS-NY APPLICATION

hi 2 all nurses..
im a nurse in the phils. and i really need help regarding my application for ny.
its about filling up the application..
~first, in the NEW YORK APPLICATION
1. what will i put in the SSN or NY state assigned number????
2. in the end part of the application, the official seal/stamp that must cover signatures, what stamp will i place??
~second, NY LICENSE APPLICATION
1.name of registration authority??? wat will i put? prc?
2.in the signature, is it my signature or the registration authority??
~third,NY TRANSRIPT AUTHORIZATION
1. is it my signature their asking or the school's registrar?
