NCLEX RN RESULT - requirement for VISA SCREEN

m confused. im about to send my verification for license form for VISA screen to CA BRN with $60 bank draft but then, i read in the internet, that i have to send the ff:
* Personal Letter request
* Request for Validation of Registration/License For VisaScreen
* Photocopy of College Diploma
* Photocopy of Transcript of Records
* Photocopy of NCLEX results
* $70 bank draft payable to BOARD OF REGISTERED NURSING
$60 - Verification of license
$10 - copy of test result
which is which? $60 and the verification form or all that is said above and $70? hope any1 could help.
