Help: CGFNS-CES for Second Courser

Are you sure? I've been searching before I posted my problem and I am using the tag "cgfns + ces" but still I can't find the answer to my question so I did post my query. If you happen to know the link of that kindly share it to me please. Thank you for your response, It serves a great help to me.

Do I have to send a “Request for Academic Records Form” to my school in college where I earned my first course which is not a healthcare related? How about the school where I attended my masteral program (also not a healthcare related), do I have to send a “Request for Academic Records Form” too?
It is written in the CGFNS-CES handbook, Item 12: Education "Send a copy of the "Request for Academic Records Form" to each school listed.
I hope somebody can help me.
