hello! need help with the cvs-ny

Hi to everyone! I am new here and i really found this site so helpful in determining my nclex plan thanks to the stickies posted by suzanne and the others... i'm from the philippines and i'm currently working out my application for ny licensure. i understand, i have to pass 3 applications (cvs-ny, ny bon applcn form 1, and visa screen certificate) but am doing and will be passing my cvs first. i'm about to have my cvs application notarized at the u.s. embassy by next week and i just have a few questions for you guys regarding my cvs application coz i want every item accurately filled out before sending it to cgfns:

1. how many authorization forms do i need to fill out? i listed 2 schools under the professional education column (as i had 2 degrees: BS Tourism and BS Nursing) plus my primary and secondary schools. Do i need to fill out 4 authorizations forms for all the schools that i attended to or just the 2 schools where i got my 2 degrees?

2. For item #11, under legal professional title, do i write there Registered Nurse and under the address/city column, i write the address of PRC (the registration authority which issued me the license)?

3. I downloaded the cvs forms and on the applicant's checklist page it stated that i should have filled out the authorization for academic records AND nursing education form. Are these 2 one and the same? coz i only have a copy of the authorization form and no nursing education form when i downloaded this...
