Filling up the CVS form


It says I need to indicate all the schools that I attended, and that I need to obtain transcripts from each of those schools. But all of my academic records have already been forwarded to my current school, so is it really necessary to obtain all those documents from my previous schools again?

Yes, you need to follow what CGFNS/ICHP is asking for.

I sent my documents via courier using the address printed o n the application form. My courier is telling me that the address is invalid. This is the address that I used:

CGFNS
PO Box 8626
Philadelphia, PA 19101-8626 USA


Also, is it OK to mail my application form and my bank draft together, or do I need to mail them separately?

Well that is the address on the CVS form but if having problems you could always try ringing them to confirm address. Which application form are you asking about if CVS then you can send both application and bank draft together

That is not the street address and courier services cannot deliver to a PO Box at a post office. You need to send it to their actual physical address, and that can be found on their website. Only the US Postal Service can deliver to a PO Box.

You do not obtain any documents for the CVS, nor do you contact anyone to have them sent to CGFNS. The requirements of NY are that CGFNS obtains all of the required documents directly. And if something is sent in that was not asked for, then it will not be used.

You only need to send in the application, the money, and the releases for them to obtain the information on your behalf. Do not send anything else at all.

Suzanne I applied for CVS using the online application form. The instructions were to print out the transcript request forms and hand them to the school so that the school can mail the transcripts directly to CGFNS. My school then gave me the tracking number for the package, and that's how I found out that the address is invalid.

I think there's a typo on the application form, because on the letter that I received from CGFNS, the address label says the zip code is 19104, not 19101. That, or the zip code changed, I don't know. Anyway, I already called DHL and gave the physical address of CGFNS.

Wait, was I not supposed to get the transcripts myself? Should I just have mailed the transcript authorization form? How is CGFNS supposed to get the transcript from my school?

Just thought CVS is all done by CGFNS they are supposed to get all documentation

- No, you do not have to send them your Transcripts of Records (TOR).

- CGFNS will obtain your Credentials for you! You just need to sign the 2 authorization forms provided in the application form.

*First authorization form titled: Authorization for Academic Records (Transcripts)

**Second authorization form is titled: "Authorization for Validation of Registration/License" (Only send this form if you are currently registered/licensed in your country.. If not registered/licensed, then you won't be needing this.)

- And of course, don't forget to send the application form itself and make sure it is signed by you and have it notarized. (Both should have the SAME date!!!)

The CGFNS Credential Verification Service for New York State independently collects and verifies the authenticity of an applicant's educational and licensure/registration credentials.



Once verified, the credentials are forwarded to the New York State Education Department to be evaluated as part of the applicant's licensure application.
