I think it's a pretty simple form to follow.
C. Which country do you have a valid RN license from? Or if you've ever held a RN license. What is the license number?
(If you don't have a license you have to say NO, if you have a license, list the countries where you have a valid license, and give them the number)
D. If you have a license in any of these countries.
(If you don't have a valid license on hand, then the answer is no)
Q16. If you've passed the NCLEX and at which location.
(you already filled it out. If you don't have a valid SS# and you took your exam in a state that requires you to
have a valid SS# before they issue you a license, then the answer is NO you don't have a license. Passing the exam doesn't automatically give you a license. The key words here are "was there a license issued". If you didn't receive a license and your name isn't on the BRN website then the answer is still NO)
