Ds 2001

Question for all you foreign nurses who have your case complete and waiting for your packet 4.

Have any of you been told by the Embassy you have to fill out a DS2001 form?

Reason I ask is, my niece is waiting for her packet 4 and when she called the Embassy to ask for an update, she was told she has to fill out this DS2001 before her case can move forward.

I never had to do this and I don't think I've heard any other foreign nurses talk about it?

Any ideas anyone?

I think the DS2001 you are referring to is the Notification of Readiness Form which is included in the Packet 3. Once you have compiled all the required documents, you have to send this form off to notify the embassy. You will not be scheduled an interview unless this form has been sent.

Am I correct in thinking the Notification of Readiness Form would/should have been forwarded to the Embassy from NVC with the packet 3?
As I said, I've never known any other nurse going through consular processing who has been asked for this.

I don't really know, as far as I remember. I had the checklist of the documents to accomplish which I sent back to the Embassy together with my DS-230 form. Then 3 months later, I was appointed an interview date.

If she was only asked for the form then she could readily download one from the internet and have it sent to the embassy. This was formerly known as the OF-169 form. Click on the link to view

As you can see it specifically says at the bottom "YOU WILL NOT BE SCHEDULED FOR AN APPOINTMENT UNTIL YOU SUBMIT THIS FORM".

There might be an updated version, just do a google search .... hope this helps.
