CGFNS-CES report issued on my maidden name and last name

Ok, I just received my CGFNS-CES report and is on my maiden name and actual last name. I called CGFNS and they said it should be fine, I called Fl Bon and they said is fine. For me it doesn't look that fine. Am I worry for nothing or should I ask for a change in CES report?
Does anybody knows if I need to apply for CVS- NY do I have to be a NY resident or I can apply from another state?
Any answer would be greatly appreciate it.

They will be used to people marrying or changing names. They will have a record of your maiden name on file so I wouldn't worry too much about it. I was married after I started CES process and had no problems with the BON I applied to

You do not have to be a resident for NY to apply but only apply if you plan on working in NY otherwise would be a waste of money
