You do not enter information into the handbook. On this page:

http://www.cgfns.org/sections/programs/ces/

there are 2 options - create an online account and submit via computer, or download the handbook, print it out and send in by mail.

If doing it online make sure you check everything is right before you submit it as any errors will cost you a delay and can only be corrected by contacting CGFNS. Probably better to do it by snail mail and know everything is right

As mentioned, if you are sending via mail, you PRINT the application form and then fill the fields by hand writing them.

if you will apply online be sure you already have the informations at hand to avoid commiting errors. i applied online and took me 1 hour to answer all the informations because i was double checking it. after 2 months i already have the report.

Print the application and then complete it and enclose it with the other documents that you are sending into CGFNS. This is always the easiest way to do things.

Illinois will not even look at your application until they have received the CES report from CGFNS, so no reason to submit your application for licensure any earlier to them.

CA has an application for licensure, just like every other state. There is no application for the NCLEX exam that you are applying for.

Anytime that there have been issues with any application sent to CGFNS, it has been with the on-line applications; once submitted, no changes can be made to it. And any date that is off or spelling of something will cause major headaches for you to straighten out.
