Agency as an employer?

Hi guys,
if you can shed some light here, I would appreciate it.
Ok here it goes.

Would it be a problem with the consular interview if I don't have a contract with a hospital?
Well my pay checks (as the agency claims) will be from them and not the hospital.
I am having doubts about it, since some say that it would be problem if the PD gets current since the consul officer will be looking for hospital contract and not the contract between the nurse and agency.
And $30 per hour salary for New York? How is it? Is it low or what? They promise a $5 increase after 1 year of employment. The drawback is, they don't offer free housing even for just 1 month.
So you could please help me with these.

Some agencies have a good program for very seasoned nurses. You may not get full time work since when the census in low the agency nurses are canceled. You may not get an benefits. There will be no orientation. And with retrogression in place you will not be able to work 5 years plus.
