Do you have a policy for standing narcotic orders?

I'm speaking of standing orders for patients with migraines that are allowed to come in 3 times a week (some patients even schedule their headache for the next day) or the frequent flyer patient with multiple doctors who all agree that narcotics are the best for her treatment plan, but she finds the one doctor to write her "standing orders" to come for her dilaidid and phenergan whenever she wishes. How do you handle this at your hospital or infusion center?
