New hospice for my home HD pt

I'm an LVN doing home dialysis. I had a pt in the hospital for 2 weeks who has been discharged home with hospice with dx liver ca.

What can i expect from the hospice nurses? I haven't seen my pts nurse yet, so i don't know if they will be around the clock or daily? She's on o2 and generally weak, but otherwise seemingly ok.

She has only so far been visited for admission, per the daughters.

What do yall think the care plan for her might be? She as of now, intends on continuing her dialysis. Comfort care and pain mgt? What else does hospice provide, oversee?

An RN case manager can visit from once a week to daily depending upon the client's needs.The visit can be anywhere from an hour to 90 mins but again this depends on the needs of the client.They may provide aides for direct care as well,same type of schedule.You will also see the rest of the team-social workers,chaplains,volunteers.As for the plan of care what would you expect?Cessation of dialysis when the burden outweighs the benefits for the client, End of life issues both physical and spiritual.....Education and support of the client and family....
