How to determine number of hours patient receives hospice aid services

Does anyone have a standard assessment tool that nurses use to determine how many hours per day or week a patient should receive hospice aide or homemaker services? Do you have a maximum number of hours per week a pt may utilize aide (not including continous care cases). Any insights or help would be greatly appreciated.
