Hourly rounding in home hospice

Are you speaking about an inpatient facility? I've worked continuous care for a couple of agencies that require half an hour or 15 minute interval charting instead of the minimal hourly charting. I think charting every 15 minutes is too much unless there is something happening that would require charting anyway. For the most part, I see hourly chart entries as sufficient for most continuous care shifts.
