- May 5, '12 by caliotter3Are you speaking about an inpatient facility? I've worked continuous care for a couple of agencies that require half an hour or 15 minute interval charting instead of the minimal hourly charting. I think charting every 15 minutes is too much unless there is something happening that would require charting anyway. For the most part, I see hourly chart entries as sufficient for most continuous care shifts.
- May 6, '12 by debblynn13Sorry I didn't make this very clear. I work home hospice and we were looking at the above article re teaching family members to do a modified version of hourly rounding to help prevent falls at home. Just was wondering if anyone had tried a modified version or not.
