Hospice resources?

Hi all!

I work in a hospice facility and am working on putting together information r/t hospice care, as a resource for staff - particularly those new to the field. Especially interested in resources on symptom management and disease specific information of the physiologic changes seen in the end stages.

Which websites/articles/books, etc. do you all use/are the most helpful?

(Also, I think I recall reading on another thread something about hospice fast facts? which website?)
