Your agency's procedure for Case Conference

    I'm confused on the best way to document Case Conferences. Does the 60 day summary cover this requirement if all members of the team participate and/or sign off on it? Is it necessary for the entire team to meet together? Meetings with the entire staff tend to run long.

    You should look to your agency policy first and decide from there. We are a small agency and one Case Manager reports on her patients each week. They are required to then put in a case conference note on each of those patients and the company has a form that is filled out for that note. THe CM's come prepared for CC and present the information and there is minimal input from the other staff. THis keeps the meeting shorter but you are correct that meeting's with the whole team run long.
