When to make the jump

I have been working on a med surg floor for over 1 year. I currently work 3 nites a week and do 8 hr shifts with everyother weekend. I recently started working for Bayada peds division, only had one nite so far with a preceptor. THey do 8 hr shifts and only have nite cases open right now. I want to eventually make the switch entirely but not sure when. I really would like to work with a home health agency where I see 5-6 pts a day but I can not find any in my area right now. Not sure if I should just leave the hospital and put all my time and efforts into Bayada and then I would have more time for HH and gain more exp or just keep doing both. I can only do a nite a week at most but lately been doing one nite everyother. I just can not work more than 2 nites in a row because I can only get about 4 hrs of sleep due to kids and hubby schedules. Any advise would be great. I am 45 and not getting any younger I want to find something I truly love and want to be at.
Thanks
