Transitional Home Care

Hi there!
Thinking of applying for a position that is called Transitional Home Care field RN. Can anyone tell me what patients "look" like in this field(Transitional home care)? Is it mostly geriatric patients with chronic conditions such as CHF, etc. or more acute patients receiving IV therapy? Can you tell me about a typical day? Again, this is transitional care at home, so it would not include care in LTC facilities. Thanks for your feedback!
