Our agency is developing a tool to help nurses give report, ie admission report to case managers or daily visit reports to case managers. Nurses were complaining of too much/too little info, so we are trying to come up with a standardized format to keep every one on the same track. Does anyone have any suggestions or templates they are using? Thanks
