PPS

With the dreaded date of Oct. !st rapidly approaching, is the any one who has developed a good screening tool for admission to homecare that they would be willing to share? I'm also looking for a letter that we could send to physicians explaining PPS and what changes they should expect. Any ideas? Thanks!
